Results from this survey will be used to provide constructive feedback regarding the department chair’s efficacy in carrying out administrative responsibilities. As you reflect on the chair's overall performance, please provide a fair and honest review of strengths as well as areas for improvement.

To encourage honesty and objectivity, you are asked not to identify yourself. Results of the survey will be summarized into one aggregate report.
